I would like to know whether the enrolment form suitable for the entire family ( i have seen similar form in Andhra Pradesh ) are available in Bangalore ? if so , are they availble in any website?
Only one address proof by the family head would do in such a case, i beleive., while individual forms for all with necessary ID s could be submitted. Any one can help us out?
